| Grade 0 | Complete paralysis |
| Grade 1 | Muscle contractions present, but no movement of limb joints |
| Grade 2 | Limb can complete the full range of joint motion, can move on the bed but cannot lift against gravity. |
| Grade 3 | Limb can complete the full range of joint motion, can lift off the bed against gravity, but cannot resist additional resistance. |
| Grade 4 | Limb can complete the full range of joint motion and can resist some external resistance, but does not reach normal. |
| Grade 5 | Completely normal |
