Classification of Perilunate Dislocations Based on Injury Severity and Location

Grade Description
1 Rupture of the radiolunate ligament or avulsion fracture of the posterior horn of the lunate, dislocated to the palmar side, convex surface facing posteriorly, concave surface facing anteriorly.
2 After rupture of the posterior ligament, the lunate rotates 270°, located at the distal anterior part, concave surface facing posteriorly, convex surface facing anteriorly.
3 With greater force, both the anterior and posterior radiolunate ligaments rupture, and the lunate dislocates to the distal palmar side of the radius, convex surface facing posteriorly, concave surface facing anteriorly. If the dislocated lunate is connected to the anterior ligament, the lunate remains viable; if both anterior and posterior ligaments rupture, necrosis may occur.
