Tossy Classification of Acromioclavicular Joint Injuries

Classification Injury Severity
Simple Sprain (Type I) Partial or complete tear of the acromioclavicular ligament, but no injury to the coracoclavicular ligament. X-ray shows slight displacement of the clavicle.
Subluxation (Type II) Complete tear of the acromioclavicular ligament with injury to the coracoclavicular ligament. X-ray shows 50% elevation of the distal clavicle, making it higher than the acromion.
Dislocation (Type III) Complete tear of both the acromioclavicular and coracoclavicular ligaments, with a significant increase in the distance between the clavicle and the coracoid process. X-ray shows complete displacement of the distal clavicle.

Explanation

The classification of acromioclavicular joint dislocations was first proposed by Tossy et al. in the 1960s. Based on physical examination findings and the extent of ligament injuries shown by imaging studies, acromioclavicular joint dislocations are classified into three types.
