Scapholunate Instability Classification

Grade I: Occult instability with partial SLIL injury, no abnormal changes on static and stress X-rays.
Grade II: Dynamic instability with partial SLIL injury and additional volar extrinsic ligament injury, normal static X-rays but abnormal stress X-rays.
Grade III: Scapholunate dissociation due to complete SLIL injury with volar and dorsal extrinsic ligament injury, static X-rays show scapholunate gap ≥3 mm and scapholunate angle ≥70°.
Grade IV: Dorsal intercalated segment instability (DISI) with secondary injury to the radiolunate ligament, scaphotrapeziotrapezoid ligament, and dorsal intercarpal ligament, static X-rays show increased scapholunate gap and angle, radiolunate angle ≥15°, and capitolunate angle ≤-15°.
Grade V: Scapholunate advanced collapse (SLAC) secondary to scapholunate joint dysfunction, characterized by degenerative arthritis of the radial styloid, radiocarpal joint, capitolunate joint, and entire wrist joint.
