Neck stiffness, moderate to severe headache, alert, oriented, no neurological deficits (except cranial nerve paralysis)

Drowsy or confused, mild focal neurological signs

Comatose, moderate to severe focal neurological signs

Deeply comatose, severe neurological deficits (e.g., decerebrate rigidity)
