Diagnosis Criteria for Hand Osteoarthritis

No. Criteria
1 Hand pain, aching, or stiffness for most of the last month
2 Bone enlargement in ≥2 of the 10 specified interphalangeal joints (bilateral second and third distal and proximal interphalangeal joints, bilateral first carpometacarpal joints)
3 Swelling in ≤2 metacarpophalangeal joints
4 Bony enlargement in ≥2 distal interphalangeal joints
5 Deformity in ≥1 of the 10 specified interphalangeal joints (bilateral second and third distal and proximal interphalangeal joints, bilateral first carpometacarpal joints)

Explanation

Diagnosis can be confirmed with 1+2+3+4 or 1+2+3+5.

Commonly seen in middle-aged and older women, with distal interphalangeal joints most commonly affected, presenting as bony enlargements on both sides of the dorsal surface of the joints, known as Heberden's nodes. If present in the proximal interphalangeal joints, they are referred to as Bouchard's nodes. After involvement of the first carpometacarpal joint, bone proliferation at the base may lead to "squared hand" deformity.
