Liver Injury Grading (AAST)

Grade Description
Grade I Hematoma: Subcapsular, non-expanding, less than 10% surface area;
Laceration: Capsular tear, parenchymal depth less than 1 cm
Grade II Hematoma: Subcapsular, non-expanding, 10%-50% surface area, intraparenchymal hematoma less than 10 cm in diameter;
Laceration: Parenchymal depth 1-3 cm, length less than 10 cm
Grade III Hematoma: Subcapsular, more than 50% surface area or expanding; ruptured subcapsular or parenchymal hematoma; intraparenchymal hematoma more than 10 cm or expanding;
Laceration: Parenchymal depth more than 3 cm
Grade IV Parenchymal disruption: Involving 25%-75% of a hepatic lobe or 1-3 Couinaud segments within a single lobe
Grade V Laceration: Parenchymal disruption involving more than 75% of a hepatic lobe or more than 3 Couinaud segments within a single lobe;
Vascular: Juxtahepatic venous injuries, i.e., retrohepatic vena cava/central major hepatic veins
Grade VI Vascular: Hepatic avulsion

Explanation

For multiple liver injuries, the injury grade increases by one level.
