Calculation Formula

MELD(i) Score = 10 x (0.957 x Loge(Serum Creatinine Value) + 0.378 x Loge(Serum Bilirubin Value) + 1.12 x Loge(International Normalized Ratio) + 0.643)

If the obtained MELD(i) score > 11, perform additional MELD score calculations as follows:

MELD Score = MELD(i) Score + 1.32 × (137 – Serum Sodium) –  [0.033 × MELD(i) Score × (137 – Serum Sodium)]

Supplementary scoring rules:

  • All values are expressed in US units (Serum Creatinine, Serum Bilirubin in mg/dL, Serum Sodium in mEq/L, INR is unitless).
  • If Serum Bilirubin, Serum Creatinine, or INR <1.0, set to 1.0.
  • If any of the following conditions exist, set Serum Creatinine directly to 4.0:
    • Serum Creatinine > 4.0;
    • Underwent ≥ 2 dialysis treatments in the past week;
    • Underwent 24-hour Continuous Venovenous Hemodialysis (CVVHD) in the past week
  • If Serum Sodium < 125 mmol/L, set to 125; if Serum Sodium > 137 mmol/L, set to 137.
  • The maximum MELD score is 40.
