Grade 1 | Superficial ulcer, no clinical infection |
Grade 2 | Deeper ulcer, often with soft tissue inflammation, no abscess or bone infection |
Grade 3 | Deep infection, with bone involvement or abscess |
Grade 4 | Localized gangrene (toe, heel, or forefoot) |
Grade 5 | Whole foot gangrene |
