Chronic Lower Limb Ischemia - TASC Classification for Below the Popliteal Artery Lesions

Classification Clinical Manifestation
Grade A 1. Single stenosis of tibial or popliteal artery, length <1cm
Grade B 1. Multiple focal stenoses of tibial or popliteal artery, each segment length <1cm
2. 1-2 focal stenoses at the trifurcation of the tibial artery, each segment length <1cm
3. Short segment stenosis of tibial or popliteal artery associated with femoropopliteal PTA
Grade C 1. Stenosis length 1-4cm
2. Occlusion of tibial or popliteal artery, length 1-2cm
3. Extensive stenosis at the trifurcation of the tibial artery
Grade D 1. Occlusion of tibial or popliteal artery, length >2cm
2. Diffuse disease of tibial or popliteal artery

Explanation

"Grade A" lesions are localized with good expected outcomes and should be treated with endovascular techniques;

"Grade B" lesions are slightly longer, but considering the risks and expected patency of surgery versus endovascular treatment, endovascular treatment is still preferred;

"Grade C" lesions have good outcomes with surgical reconstruction, but for patients with high-risk factors, minimally invasive endovascular techniques can be attempted;

"Grade D" lesions should be treated surgically.
