Sedation-Agitation Scoring

Grade Description
7 Dangerous agitation. Pulling on the endotracheal tube, attacking medical staff, climbing over bed rails.
6 Very agitated. Unable to calm down, requires restraint, biting the endotracheal tube.
5 Agitated. Attempting to sit up but can be calmed with verbal prompts.
4 Calm cooperation. Follows instructions.
3 Sedated. Difficult to awaken, can follow simple commands.
2 Very sedated. Responsive to stimulation, unable to follow commands.
1 Unawakenable. No or only minimal response to painful stimuli.
