Grade	Description
1	Fully awake and oriented.
2	Drowsy but eyes open.
3	Somnolent, eyes closed, but can be awakened by command.
4	Somnolent, eyes closed, but can be awakened by mild physical stimulation (e.g., pulling the earlobe).
5	Mild physical stimulation cannot awaken.
