Alberta Croup Severity Classification for Pediatric Croup

Mild Occasional barking cough, no wheezing at rest, and no mild retractions of the sternum and/or intercostal spaces.
Moderate Frequent barking cough, wheezing is easily heard at rest, retractions of the sternum wall and chest wall at rest, but little or no respiratory distress or agitation.
Severe Frequent barking cough, significant wheezing on inhalation, and wheezing occasionally heard on exhalation, marked retractions of the sternum wall, significant respiratory distress and agitation.
Critical Respiratory Failure Barking cough (usually not obvious), wheezing may be heard at rest (occasionally very hard to hear), retractions of the sternum wall (may not be obvious), drowsiness or altered consciousness, cyanosis of skin and mucous membranes may be seen without supplemental oxygen.
