Calculation Formula

For males: Creatinine Clearance = (140 - Age) × (Weight) / (72 × Creatinine);

For females: Creatinine Clearance = (140 - Age) × (Weight) / (72 × Creatinine) × 0.85;

Obese individuals should convert their weight to Ideal Body Weight (IBW) for calculation. The Devine formula is as follows:

  • For males: IBW, kg = 50 + [ 2.3 × (Height, inches – 60) ]
  • For females: IBW, kg = 45.5 + [ 2.3 × (Height, inches – 60) ]

Adjusted Body Weight (ABW, kg) = IBW, kg + 0.4 × (Actual Weight, kg – IBW, kg)

Interpretation

This formula is the most commonly used to determine creatinine clearance, usually for estimating Glomerular Filtration Rate (GFR). A value lower than 90 mL/min indicates some degree of renal dysfunction, while a GFR lower than 60 mL/min is considered chronic kidney disease. The clearance rate of less than 15 mL/min indicates End-Stage Renal Disease (ESRD) requiring dialysis.
